In this session hosted by NCI’s Healthcare Delivery Research Program, the speakers will discuss how electronic health record–enabled mobile health IT can be leveraged to identify social risks among individuals diagnosed with cancer and then matched with social support services using an intelligent community referral system. Speakers will describe how leveraging health IT can improve the efficiency, effectiveness, and sustainability of efforts that address social risks as a regular component of healthcare delivery.
