This webinar, presented by David Poms of the DC Primary Care Association, focused on the need for social determinants of health screening tools that screen and link patients to social services. Mr. Poms summarized the process of implementing a citywide/regional social needs community-clinical linkage solution; described the progress of a pilot project in Washington, DC and its preliminary outcomes; and addressed potential resources to support other regions in care coordination to optimize social support for individuals at highest risk of poor health due to unmet social needs.
