The primary aim of this report is to describe the use of publicly available county-level data on the healthcare workforce, chronic disease prevalence, and the socioeconomic environment of rural communities to create a metric that will allow for the identification of rural counties based on how under-resourced and disadvantaged they are. Secondly, this report aims to describe the sociodemographic and health related characteristics of identified high need rural counties.
